The fee waiver process has changed from previous years. Due to new legislation in our state, the following forms have been revised. To qualify for a fee waiver, fill out the attached forms and return for confirmation of eligibility. In addition to these forms, you must bring a payroll check stub or tax information to be eligible. Fees will not be covered until you are approved.
